Goldenheart Day Care – Child Care Program Reporting Policy

Goldenheart Day Care follows Minnesota Statute 245A.145 and applicable childcare licensing
requirements regarding reporting incidents, injuries, emergencies, and suspected maltreatment. This
policy ensures the safety of children and clear communication with parents and licensing authorities.

  • Serious injury to a child requiring medical treatment by a doctor.
  • Death of a child while in care.
  • Suspected abuse, neglect, or maltreatment of a child.
  • Any emergency situation affecting the safety of children in care.
  • Parents or guardians will be notified immediately of any injury, illness, or incident involving their child.
  • A written incident report will be completed when necessary.
  • Parents will be informed of any medical care provided or recommended.
  • CPR/First Aid–certified staff.
  • Goldenheart Day Care will report serious incidents and maltreatment concerns to the appropriate licensing agency when required.
  • Reports will be made within the timelines required by Minnesota childcare licensing rules.
  • All incidents will be documented and kept in program records.
  • Documentation may include incident reports, communication with parents, and follow-up actions.
  • Records will be maintained according to childcare licensing requirements.
  • A tour and onboarding process designed for clarity, comfort, and confidence.
